For your first appointment, all we’ll need is proof of insurance (i.e. your insurance ID card) and an initiating prescription from your physician stating your diagnosis with an order for an Orthotist or Prosthetist evaluation. You can either bring those with you to your first appointment, or you can fax it to us when you schedule and we will pre-qualify you for coverage.

Your first appointment will usually be just an evaluation in order to determine the correct treatment plan to facilitate your personal rehabilitation. All evaluation and fitting visits are covered under the payment for your device, so don’t hesitate to contact us with any questions or concerns you might have.

After the initial evaluation and consult with your practitioner, we will contact your physician to collect any medical documentation your insurance requires. They will usually require a visit note from your physician describing your evaluation and condition justifying the device ordered, as well as a detailed written order or CMN which we will provide. Some insurance plans require a prior authorization as well which will delay the initial process from a few days up to 6-8 weeks depending in your insurance. We will keep you updated throughout the process so you will be aware of any progress or unexpected delays. For some devices, we might be able to offer you a loaner or trial device from the manufacturer on an as needed basis. Feel free to ask one of our team members if this is an option.

After all documentation has been received by our staff, we will release the order to be fabricated and O & P devices will be completed within 10-15 business days. For customized wheelchairs, the timeline will be communicated to you through your ATP. Some orthotic devices which are urgent can be completed within a few days, or even within 24 hours, on an as needed basis.

As soon as we have a completion date, we will contact you to schedule your fitting.
